The Mood Disorder Questionnaire (MDQ) is a brief, self-report screening tool designed to identify the presence of bipolar spectrum disorders (including Bipolar I, II, and NOS). It consists of 13 yes/no questions regarding manic/hypomanic symptoms, plus questions on symptom clustering and functional impairment.
